Telephone Calling Card

I have a calling card from Verizon that consists of name, phone number, card number and PIN, and I really need to enter this into 1Password. There do not appear to be any options for creating a wallet item with custom fields. How can I get this information into my 1Password database?

    Hm. There certainly isn't a "Calling Card" template, but if it were me, I would probably just enter it as a "Reward Program" item.
