Username fields
I've noticed that 1Password 3.8.20 for Mac frequently gets the username wrong. Most often, it uses an email address as the username, even when there is a field called "name" or maybe even "username". Please look into that. Thanks.

This should be resolved in the next extension beta. We actually just made a change in exactly this regard.
I don't have a time frame, but you can switch to the beta channel by installing the current beta extension, so you will get the next one as soon as it is available. Or just wait for the next stable build.
To install the beta extension, just check "Allow beta extensions" on the extension download page before installing the extension(s):

It should be less of an issue if you are saving Login items via the extension on the actual login page of each site. It mostly rears its head if you save a Login item on a registration page and then try to use it to fill a login page where the form fields are often different.
