A suggestion for future versions: Provide a set of individual fields for each item and allow users to change field labels, similar to SplashID.

    We're hoping to add exactly this type of functionality in the future, we want you to be able to create your own templates within 1Password. I can't promise exactly when it will be available, but it's very much on our radar.
