Password Generator missing refresh button

When using the 'Generate' button while editing or creating a login, the Refresh button, which used to be on the right hand side of the password field is missing. However, I can click where it [i]used to[/i] be and still generate a new password but the icon itself is invisible. See the attachment for a screenshot.
