I can't generate a Diagnostic Report either, so something's pretty broken...
I removed 1Password from Applications, downloaded the latest release build, and ff4 integration is working fine. I just upgraded to FF4 last night and here is my challenge:
I'm running Mac OSx 10.5.8
1Password 3 (http://screencast.com/t/xxoqQG7mWe)
However when I run a diagnostics it say's I'm running 1Password Version: 2.12.1 (build #7671)
I've checked with 1 Password and it says I'm running the most recent update available and "thanks for staying update)
This alone is a little confusing because I understand FF4 only works with 1password 3.
However I'm not getting the FF4 icon as you can see http://screencast.com/t/pcXoIkf3x1 in order to check it. (Or chrome for that matter)
I restarted FF4 and checked to add extensions and no go.
Am I missing a step?
The 1 password icon doesn't show on my Safari either and it doesn't give me the option to add chrome either like I see in some of the other screen shots.
Is this user error or is something just bugged?

I'm sorry for the trouble here. The screenshot you sent shows your license window, which does show a 1Password 3 license, but as the diagnostics report says you're running 2.12.1 then this is the version you're running.
The reason you're told that you're running the latest version already is because the upgrade to 1Password 3 can often require a data file upgrade at the same time, and so we couldn't make the process automatic.
With that said, and because you do have a valid 1Password 3 license, here are complete upgrade instructions, please make sure you follow each step. Most of the time, it's a simple omission of one of the steps that causes an issue, so going through them one by one will take care of any problems.
1. Download 1Password 3 from our web site (http://agilewebsolutions.com/downloads), but don't install it yet.
2. Run 1Password 2.12.1, the old version you've been using. If you don't have it anymore, you can download it again from the same downloads page above - it is listed at the bottom of the page.
3. In 1Password 2, select 1Password > Preferences > Keychain.
4. Look at the button at the very bottom. If it says “Switch Back to OS X Keychain”, then you are using the new data format and can proceed to step 5. If, however, the button says “Switch to Agile Keychain”, please click that button and follow the instructions. You may be prompted for access to each item; in this case, please click Allow or Always Allow for each one.
5. Quit 1Password and any running web browsers.
6. Open your Applications folder.
7. Drag 1Password 2 to the Trash. This will not harm your data.
8. Install the 1Password that you downloaded in step 1.
9. Run 1Password and then your web browser.
That should get things up and running for you, and because you're installing a fresh copy of 1Password 3 the Firefox 4 extension should be enabled by default.

I have tried all of the suggestions and still cannot get FF4 integrated with 1Password. I never knew I would miss 1Password so much...can someone please help?
When I tried to install 1password from ff4 extensions -> Install from File -> 1Password - I get "This add-on could not be installed because it appears to be corrupt" - I have downloaded the latest upgrade (3.5.9) from the agile site...

And here's the rest of my solution:
After following FFR4's guidelines for a clean install (google it), I opened my copy of the OLD profile and removed every file in it that referred to an extension I had installed in FF3.
I then created a new Profile (using the Profile Manager, as in the instructions for a clean install) and copied into that folder all the remaining files in the old profile. After selecting the modified old profile, I restarted FF4.
Now, in 1Password Preferences, FF4 appears twice, once with each profile. I installed the browser driver into the modified old profile and restarted FF4.
Now everything works, AND I have my old bookmarks, logins, etc. back.
Apparently, there was something in the Profile left over from earlier versions of FF that prevented the new 1P from loading properly.

Wow, that certainly is quite a bit of work, but glad to hear you were able to get it sorted out.
My solution was much simpler than that....I dragged 1password into the trash, reinstalled a clean copy, then went to FF Tools -> Addons -> Extensions -> then I went to Install addon from file, chose the clean 1Password App, closed FF, reopened it and all is well.

So I believe the problems with installing the Firefox 4 extension are specifically related to 1password 3.6.0-Beta2. I haven't been able to get the extension installed no matter what I tried (including completely resetting my Firefox profile and reinstalling firefox itself. So I tried deleting 1password and installing the release 3.5.9 version and I was able to enable the firefox 4 extension with no issues. Then I removed the extension and updated back to 3.6.0-Beta2 and couldn't install the Firefox 4 extension again.
Hopefully this is helpful information for you guys ( I generally let my computer run the beta versions of 1password).

Thanks for the heads up Rob. We actually have fixed some Firefox 4 issues in the latest beta version, so that comes as a surprise to me. If you can email us a Diagnostics Report from 3.6.0.BETA-2, that would be immensely helpful.

I'm having a similar issue, but the fix isn't working. When I try and check the Firefox 4 box in 1Password > Preferences > Browsers is does not allow me to do so? Any ideas? I really want my 1Password toolbar back in my browser.
I've also tried moving back to Firefox 3.16.6 but even that is not working (with the Firefox 3 box in 1Password > Preferences > Browsers checked.

I'm sorry for the trouble here, what you're describing sounds like your 1Password installation may be damaged.
The best thing to do is to manually download the latest version from http://agilewebsolutions.com/downloads and then reinstall. Make sure you quit any browsers and drag the current version of 1Password to the trash before installing the newly downloaded version. This won't harm any of your 1Password data as it's stored in a separate file which doesn't get touched by a reinstall.
Once you've done that, let's try reinstalling the relevant Firefox extension for 1Password:
1. Make sure Firefox is not running.
2. Launch 1Password from your /Applications folder
3. Click 1Password > Preferences > Browsers
4. Uncheck the box next to all versions of Firefox
5. Launch Firefox and click Tools > Add-ons > Extensions. Make sure that 1Password is *not* listed.
6. Step #5 is the most important step so I'll repeat it again here <img src='http://forum.agile.ws/public/style_emoticons/<#EMO_DIR#>/smile.gif' class='bbc_emoticon' alt=':)' /> Launch Firefox and make sure 1Password is not there.
7. If 1Password is appearing in the Extensions window, click the Uninstall button on the 1Password extension and quit Firefox.
8. Return to your 1Password window and check the box next to Firefox. There are two versions: 3 and 4. Pick the one that you have installed.
9. Restart Firefox and click Tools > Add-ons > Extensions. Make sure that 1Password is listed.
That should get you up and running, so please do let me know.
